It’s called sciatica because it involves irritation or compression of the sciatic nerve—the largest nerve in the body, running from your lower back through the hips and down to the feet. In some cases, this nerve gets pinched not at the spine but by a tight piriformis muscle deep in the glutes, a condition known as piriformis syndrome. The symptoms often feel the same: pain, numbness, tingling, or weakness anywhere along the nerve’s path.

For some, the pain stays mostly in the lower back and only shoots down with certain movements; for others, there’s no back pain at all—just burning or aching in one leg. It might only flare up when sitting or standing too long, or it might be constant and exhausting, with relief coming only when you lie down.

At Cucamonga Acupuncture & Wellness, we specialize in treating sciatica at every stage—from mild, nagging irritation to severe, debilitating pain caused by a herniated disc. The story often unfolds the same way: first, you visit your primary care doctor and are prescribed painkillers or muscle relaxers to dull the pain. When that only brings short-term relief, you’re referred for a steroid injection or physical therapy—the injection may last longer, and PT can help restore proper movement through the hips and spine. Some also try chiropractic adjustments, which can help if done correctly.

But when those measures don’t work, the next recommendation is often surgery. Unfortunately, this is where things can get complicated. Studies show that up to 40% of back surgeries fail to relieve pain long-term, leading to what’s known as Failed Back Surgery Syndrome—a condition that can create more scar tissue, instability, and chronic nerve irritation.
